tiered storage doesn't support verify or salvage

      Verify and salvage don't work (yet) on tiered: objects.  This ticket will update the tiered storage implementation to work with our existing verify and salvage code.  This will entail changing the system from having a 1:many mapping of WT_BLOCK to WT_FH into a 1:1 mapping.
