Debug dumps don't dump page deletion info

      There should be code in debug_cell_int to print the deletion information in WT_CELL_ADDR_DEL cells, and code in debug_ref/debug_ref_state to print the page_del information for deleted or instantiated refs that have it.

      Not having this will sooner or later cause confusion.
