The MongoDB manual annotates API changes with italicized text (e.g. "New in version 2.6"). We should do the same for methods and options that were introduced in each minor X.Y release.

We don't have any published docs for 1.0.x (PHPLIB-236 exists, but may never be resolved), but we should start doing this for 1.2.0 docs.
