Any time a server is removed from topology the driver should log a message with warn log level. The server removal events are almost always significant - they either indicate a misconfiguration or, in case of elections/failovers/upgrades, they provide useful intel for troubleshooting should anything go wrong.

When a server is discovered by SDAM but not added to the topology, a message should be logged at warn level as well for the same reasons.
