Generate table and index idents in a way that doesn't require storing state

      Currently we generate WT table idents using the combination of a counter and random suffix in DurableCatalog.

      This makes it difficult to move ident generation elsewhere, results in somewhat complicated code, and might not work properly for replicated idents.
