Unit tests block_ext 2 - Insert functions without block

      Create unit tests for the block manager's extent insert/remove functions without a block parameter.

      __block_off_insert
      __block_ext_insert

      Error testing, if applicable for the function, are not included. I wanted to get the normal tests done for all before adding error testing.
